Linden House is dedicated entirely to people living with dementia. Everything within the home is organised around how dementia is experienced day to day, so support remains consistent and surroundings are easier to understand.

Responsibility sits clearly within the home. Decisions are made by experienced leaders and clinical professionals who know the residents well.

Oversight is continuous, allowing changes to be recognised early and addressed calmly.

Dedicated dementia care

Care at Linden House is delivered within a home designed and operated exclusively for people living with dementia. The environment, routines and leadership are structured to reduce confusion and support wellbeing. Because the home is dedicated to dementia, teams are not balancing competing care models. Support remains consistent, communication is clear and daily life is organised in ways that are easier to follow, creating the conditions for a good quality of life.

Nursing care

Many residents at Linden House require ongoing clinical input as part of their dementia care. Nurses are present within the home and work closely with care colleagues so health needs are managed alongside daily life. Health changes are recognised early because teams understand each resident’s usual patterns. Care plans are reviewed regularly so support remains appropriate as needs evolve.

Admiral Nurse support

Linden House is supported by Colten Care’s in-house Admiral Nurse, working in partnership with Dementia UK. Our Admiral Nurse provides additional dementia guidance to families and colleagues within the home, offering deeper understanding and practical advice as needs evolve.Residents continue to be supported by the team who know them well, with specialist insight available to strengthen care and provide reassurance when needed.

End of life care

End of life care at Linden House focuses on comfort, dignity and personal wishes. Support is guided by clinical experience and clear communication with families. Residents remain in familiar surroundings, supported by people who know them well. Adjustments are made carefully so care remains steady at this stage.

Short stays and respite care

Short stays are available for planned respite or as a way to experience life at Linden House before making longer-term decisions. Residents on a short stay receive the same assessment, planning and oversight as permanent residents and are fully included in the life of the home.

Hospital discharge support

Linden House supports people who are medically ready to leave hospital but require dementia care before returning home or making longer-term arrangements. Assessment is carried out promptly, and support is in place from arrival. Recovery and decision-making take place within a structured environment designed specifically for dementia.

Care shaped around the individual

“All About Me”

“All About Me” guides how care is planned and delivered at Linden House. Support is shaped around the person’s history, preferences and ways of communicating rather than applied uniformly.

This shared understanding informs everyday decisions, from how mornings begin to how reassurance is offered during unsettled moments. As dementia progresses, care adapts in a measured way, guided by what is known about the individual.
