Daily routines restore structure after the disruption of a hospital stay, and support is assessed promptly so the right level of care is established without delay. This creates a clear step between hospital and home, allowing recovery to continue while decisions about the future are made with clarity.

Support designed around recovery
Following discharge, residents are welcomed into the home and supported as part of daily life. They have their own room, enjoy meals prepared each day by our own chefs, and benefit from the stability of a well run environment. Housekeeping and maintenance are provided by in house teams.
Support is planned around individual needs and reviewed as recovery progresses, so it adapts as circumstances change. Where required, teams coordinate transport from hospital, including working with specialist patient transport services to support a smooth and dignified arrival.

Teams work closely with hospital discharge teams, GPs, and other healthcare professionals to support a smooth transition from hospital. Information is shared clearly so that support remains coordinated and well understood.
